By purchasing a device directly from 11 Health You agree to the following statement: I consent and agree to 11 Health ("the Company") receiving, collecting and storing anywhere it considers appropriate (whether within or outside the United States of America) all of my personal health information that is transmitted or communicated by the ostom-i Alert Sensor and to the Company's sharing any or all of my personal medical information with or delivering it to all physicians, hospitals, clinics or other persons may designate at any time to the Company. I understand that in its storage or disclosure of my personal medical information, the Company will comply with all applicable Federal, state and local laws.

Given to, signed by, and returned to 11 Health by, any intermediary purchaser from 11 Health, such as a hospital, physician or other person who will resell the device to an ultimate patient user: The undersigned ("Purchaser") acknowledges and agrees that it will comply with all Federal, state and local laws, rules and regulations applicable to (i) Purchaser's sale to any patient or other person of ostom-i Alert Sensor, (ii) 11 Health (the "Company") receiving, storing anywhere in the world (whether within or outside the United States of America) any and all of the patient/purchaser's personal health information that may be transmitted by the device(s), and (iii) the Company's sharing any or all of such personal medical information of the patient/user with or delivering it to any or all physicians, hospitals, clinics or other persons such patient/user may designate at any time. Without limiting the generality of the foregoing, Purchaser will give to each patient/user all necessary or appropriate written notices, and will obtain from each patient/user all necessary or appropriate consents and agreements signed by such person, and it will retain the same for the period required by applicable law, and will also promptly deliver to Seller a signed copy of all such notices and signed consents and agreements.

Each time you use the Website and/or Applications, the then-current version of this Privacy Policy will apply. Accordingly, each time you use the Website and/or Applications you should check the date of this Privacy Policy (which appears at the end) and review any changes since the last time you used the Website and/or Applications. 11 Health does not solicit or knowingly collect personally identifiable information from children under the age of 13. If 11 Health obtains actual knowledge that it has collected personally identifiable information from a child under the age of 13, 11 Health will immediately delete such information from its database. A unique device identifier is a string of alphanumeric characters (similar to a serial number) that is unique to each mobile phone or other wireless communications device. 11 Health uses Mobile Device Data to understand usage patterns and to improve the Applications.
